DESCRIPTION OF PATIENTS RIGHTS AND RESPONSIBILITIES

The NHS Constitution establishes the principles and values of the NHS in England. It sets out rights to which patients, public and staff, are entitled and pledges which the NHS is committed to achieving, together with responsibilities which the public, patients and staff owe to one another to ensure that the NHS operates fairly and effectively. All NHS bodies and private and third sector providers supplying  NHS services are required by law to take account of the Constitution in their decisions and actions. A copy of the Constitution is available from the Practice by request.

HOW THE PRACTICE USES PERSONAL INFORMATION

We respect your right to privacy and keep all your health information confidentially and secure. It is important that the NHS keeps accurate and up to date records about your health and treatment, in order that those treating you can give you the best possible care and advice. This information is only available to those involved in your care and you should never be asked for personal medical information by anyone not involved in your care. Disclosure of identifiable patient information to any other outside agencies will only be given after receipt of written permission from the patient, dated within three months of request. You have the right to know what information we hold about you from time to time, you will be asked to prove your identity prior to seeing your records, please contact the Practice Manager.

However, if the doctor thinks that by giving you certain information held on your records may be harmful to you, or affect your wellbeing, he or she has the right to withhold this information.  When you ask for a copy of your health records, there may be a charge for administration, again please contact the Practice Manager for details or ask the receptionist for details.

ZERO TOLERANCE POLICY

The practice has a zero tolerance policy and any patient found to have verbally or physically abused any member of staff or is found to be upsetting other patients, will be removed from the practice list.
